Every church goes through phases of development. The church as described in Acts went through at least three distinct life cycles. This moving target is not that difficult to understand, but it takes the input of several people that have an awareness of the local church to define where you are in your current life cycle.

This survey includes several items for which you are to respond with varying levels of agreement. Answer each from your perspective. Don't rush, but don't linger too long either. Your first instinct is probably your best answer.
